Our office schedules appointment times specifically for you and your particular dental needs. We make every effort to respect your time by staying on schedule. If you must change an appointment time, we require at least a 24 hour notice so that we can offer that appointment time to someone else who is waiting for that appointment time. There will be a charge of $25.00 for missed appointments or appointments canceled without 24 hours notice. We reserve the right to dismiss a patient after three failed appointments.
Dental insurance coverage is a benefit that your employer may provide for you and your family. It is a contractual agreement between the patient, the employer and the dental insurance carrier. It is not a contract between our dental office and your dental insurance carrier. Dental insurance policies vary in covered procedures, annual maximums and deductibles. As a convenience to our patients, our office will file your dental insurance claim with your carrier. However, you are responsible to pay the deductibles and the patient portion of dental fees at the time of service and you are responsible to pay all charges if your dental insurance does not pay within 45 days of the date of service.
1. Cash or Check at time of visit. (A $30 charge is made for all returned checks.)
2. Mastercard or Visa credit card payment at time of visit.
3. 50% of fee paid at time of service for prosthesis (crown, bridge, partials, dentures, etc.); the remaining 50% paid at delivery of prosthesis. Delivery is subject to full payment.
4. Three (3) monthly payments (with approval of Business Manager). One third of payment at the time of service, one third within 30 days, and one third within 60 days.
